Better connected care through My Health Record
My Health Record is a safe and secure place to keep your health information online. It can be accessed by you and your healthcare providers quickly and easily anytime, including in an emergency.
As of 9 December, you can share your My Aged Care support plan with My Health Record.
Your support plan is a record of everything discussed in your aged care assessment and includes the care and services that will best help you.
During your assessment you will be asked to authorise sharing your support plan with My Health Record.
This will give health professionals, such as your doctor, a better understanding of your aged care needs and help them provide you with better care.
You can access My Health Record from your myGov account to choose who can see your support plan. You can also make changes by calling the My Aged Care contact centre on 1800 200 422.
