Make your choices matter

It's important to think about what kind of healthcare you would want if you couldn't make decisions for yourself.
National Advance Care Planning Week, 17 – 23 March, is the perfect time to have a conversation with your loved ones and healthcare providers about what matters to you.
Advance care planning involves planning for your future healthcare. If you get very sick and can't communicate or make decisions, who would you want to make decisions for you? What would you want them to do?
Advance care planning is about making your wishes and preferences known, in case that time ever comes.
The best place to start is to have a conversation. Talk to your loved ones and your GP or healthcare provider. This is particularly important if you have a serious health condition, as it will help you understand the kinds of decisions that might need to be made in the future.

Let's change aged care, together
Every Australian should feel confident about accessing high quality and safe aged care, when and where they need it.
The changes mean Older people will have greater choice and control, services that respect and meet their needs, and support to stay independent as they age.
