On-site pharmacists in aged care homes

From 1 July 2024, aged care homes can have pharmacists on-site. This provides better oversight of medication use and will reduce the risk of medication related harm.

On-site pharmacists will:

  • regularly review medications
  • meet with people living in aged care homes and their families
  • work closely with medical professionals and aged care staff on the safe and effective use of medicines.

This government initiative provides funding for community pharmacies and aged care providers to employ on-site pharmacists in residential aged care homes. We want to give residents and their families access to expert advice and ensure they are getting the best care possible.

Having an on-site pharmacist in an aged care home will:

  • improve medication use and safety, including the use of high-risk medications
  • provide continuity in medication management, such as day-to-day review of medications and prompt issue resolution
  • provide easy access to pharmacist advice for residents and staff
  • improve coordination with local general practitioners, nurses and community pharmacy
  • improve understanding of residents' medication needs.
