Support for people living with dementia to move from hospital to aged care

Moving from hospital to aged care can be a big change, especially for older people living with dementia. We have a program to make this transition smoother.
The Hospital to Aged Care Dementia Support Program can help you move from hospital into a residential aged care home or back into your own home.
The program supports hospital and aged care staff, older people, their family and carers:
- during the hospital stay
- when transitioning out of hospital
- while settling into aged care for up to 3 months.
After leaving hospital, intensive support will be offered to ensure a successful, safe and smooth transition into aged care.
The program is being delivered in 11 locations, with a presence in all states and territories. Support is already available in Adelaide and Hobart. Other locations will become available in the coming months.
The hospital will initiate the referral and submit it to Dementia Support Australia.
People living with dementia often stay in hospital longer than other people. This program aims to improve outcomes for older people living with dementia who are at risk of delayed hospital discharge.
